Top Tier Texas K9 reserves the right to immediately change your dog’s type of boarding/training if we believe it is necessary to protect the health and well-being of your dog, other dogs, or a staff member.

Top Tier Texas K9 cannot guarantee that toys, blankets, or bed will be kept in the same condition as brought in. Top Tier Texas K9 provides necessary bedding, bowls and chew toys. Please do not bring these items.

Dog owner understands the risks involved with communal boarding of dogs. Our boarding and training dogs play and/or train in the yard and inside around other dogs. Although we will offer reasonable care, the unpredictable personality of dogs can sometimes lead to illness or injury.

If in our judgment, your dog requires medical care, you agree to be solely responsible for the payment of all medical bills for your dog and you release Top Tier Texas K9, its officers, directors, agents, and employees of and from any and all responsibility for, or claims, damages, or debts arising out of or related to such medical care, including but not limited to, transportation to/from the veterinarian clinic and choice of veterinarian or animal hospital.

In the unlikely event of a medical emergency, I authorize Top Tier Texas K9 to perform any necessary medical procedures, treatments, medications and medical decision-making for the well-being of my dog(s).

I agree to pay Top Tier Texas K9 with the credit card on file or settle upon Board & Train pick up all of the pet charges incurred in the emergency treatment to include procedures, treatments, medications and any medical decisions made to ensure the wellbeing of my dog(s).

By signing this authorization I do herein give Top Tier Texas K9 permission to act as my agent to authorize the veterinarian of Top Tier Texas K9 choosing to provide or perform necessary medical procedures, to medicate and make any additional medical decisions directly related to the medical emergency involving my pet(s) during their Board & Train stay with Top Tier Texas K9

Dog owner understands the possible effects of stress from a new environment on a dog can include intestinal problems, including decreased appetite and diarrhea.

ASSUMPTION OF RISK AND WAIVER OF LIABILITY

I understand that Top Tier Texas K9 training program incorporates real life distractions, exposure and socialization for my dog. During my pet’s stay there may be situations where he/she will be traveling in a moving vehicle and/or training outside of the training facility.

I understand that all dogs are trained by ‘Top Tier Texas K9’ without liability on its part for loss or damage by or from disease, death, running away, theft, fire, or injury to persons, other dogs, or property by or to said dog(s), or other unavoidable causes, due diligence and care having been exercised by Top Tier Texas K9

I have read all the provided documentation and fully accept responsibility for placing my dog in the care of the Top Tier Texas K9's Board & Train environment.
